- Attitudes toward patient-centred care, empathy, and assertiveness among students in rehabilitation areas: a longitudinal studyPublication . Grilo, Ana; Vinagre, Graça; Santos, Margarida Custódio dos; Martinho, Joana Ferreira; Gomes, Ana IsabelThis study assessed attitudes toward patient-centred care, empathy, assertiveness, and subjective perception of communication skills and technical knowledge among Portuguese undergraduate students in healthcare. These students may develop rehabilitation activities with patients in their person-oriented or technique-oriented professions. Portuguese nursing and allied health students from two public higher education schools completed questionnaires in the first and third academic years: Patient-Practitioner Orientation Scale, Jefferson Scale of Physician Empathy, Scale for Interpersonal Behaviour, and subjective perception of technical knowledge and communication skills. A total of 183 students completed the surveys. In the first year, students showed moderate to high scores on patient-centredness attitudes, empathy, and assertiveness and perceived themselves as having good communication skills. Students from person-oriented programmes significantly improved their Total and shared patient-centred attitudes in the third year compared with students attending technique-oriented professions. Significant differences in empathy were found between groups in the third year. Distress associated with assertive behaviours increased significantly across time in students from technique-oriented programmes compared with their peers in person-oriented programmes. The results suggest that the health profession’s orientation and the programmes’ specific curriculum might have a role in how some dimensions evolved in the two groups of students. The increasing assertiveness-related discomfort highlighted the importance of assessing and monitoring students’ emotional well-being during their initial interactions with patients.
- Avaliação da centração no doente em estudantes de enfermagem e enfermeiros portuguesesPublication . Grilo, Ana Monteiro; Gomes, Ana Isabel; Santos-Rita, Joana; Santos, Margarida Custódio dosIntrodução: A abordagem centrada no doente tem sido valorizada no meio académico e profissional; contudo, parecem existir dificuldades em aplicá-la à prática clínica. Estudos mostram que os cuidados prestados pelos enfermeiros são sobretudo instrumentais, baseando as suas ações em necessidades inferidas e não confirmadas pelos doentes. Esta realidade pode estar relacionada com o tipo de metodologias de ensino usadas nos treinos comunicacionais ou com barreiras pessoais e profissionais percebidas pelos profissionais. Objetivos: O objetivo principal deste estudo é avaliar o tipo de orientação assumida por enfermeiros e estudantes de enfermagem na prestação de cuidados de saúde ao doente. Pretende-se também estudar se existem diferenças: a) entre sexos, b) em função do ano de escolaridade dos estudantes, c) entre estudantes e profissionais de saúde.
- Centração do paciente em estudantes de medicina e enfermagem: implicações para os curricula académicosPublication . Grilo, Ana Monteiro; Santos-Rita, Joana; Santos, Margarida Custódio dos; Gomes, Ana IsabelA centração no paciente pode ser definida como “o cuidado que é congruente e responsivo às necessidades, desejos e preferências dos pacientes”. A centração no paciente tem vindo a ser, consistentemente, associada à satisfação do paciente, ao aumento da adesão ao tratamento, ao mais rápido restabelecimento do doente, a menor perturbação emocional e à diminuição dos erros médicos. Por esta razão, é hoje considerada uma competência fundamental a desenvolver na formação de futuros profissionais de saúde. O objectivo deste estudo é avaliar as atitudes de centração no paciente em estudantes de medicina e enfermagem portugueses (integrados em anos académicos préclínicos e clínicos). Participaram 369 estudantes de uma faculdade de medicina de Lisboa e 524 estudantes de enfermagem de uma escola superior de enfermagem também de Lisboa, que preencheram a Patient‐Practitioner Orientation Scale (PPOS). Na última década, a PPOS tem vindo a ser utilizada internacionalmente em diversas investigações relativas à educação médica o que constitui um bom indicador da sua aplicabilidade. Estes estudos têm permitido reflectir sobre as metodologias de ensino que centrem o estudante de saúde no objecto central da sua acção: o paciente. Nesta comunicação são comparados os resultados dos anos pré‐clínicos e clínicos dos estudantes de medicina e enfermagem e discutidas as necessidades de re‐orientações educativas que promovam a centração do paciente, particularmente nos curricula das escolas de medicina.
- Centração no paciente: contributo para o estudo de adaptação da Patient-Practitioner Orientation Scale (PPOS)Publication . Grilo, Ana Monteiro; Rita, Joana S.; Carolino, Elisabete; Gomes, Ana Isabel; Santos, Margarida Custódio dosObjetivo: Este estudo teve como objetivo traduzir e contribuir para a adaptação para a população portuguesa (Português Europeu) da Patient-Practitioner Orientation Scale (PPOS). Método: Após o processo de tradução e de pré-teste, a escala foi aplicada a 593 estudantes do 1º ao 6º ano do curso de Medicina em várias Universidades de Portugal Continental. A validade do construto e a fiabilidade do instrumento foram aferidas através da análise fatorial exploratória (ACP) e confirmatória (AFC), e do cálculo do coeficiente alpha de Cronbach. Resultados: A versão final explica 31.54% da variância total e confirma a estrutura em dois fatores: Caring, (19.56% da variância) e Sharing (11.98% da variância). Os itens 2 e 4 apresentaram inconsistências com os fatores definidos à priori (versão original do instrumento), os itens 9 e 17 obtiveram cargas fatoriais inferiores a .3, e o item 3 registou uma diferença inferior a .1 entre as cargas fatoriais para os dois domínios. Os coeficientes de alpha de Cronbach foram .65, .50 e .56 para a escala total, e subscalas Caring e Sharing, respetivamente. A AFC revelou um bom ajustamento global do modelo de medida (χ2(132, N = 593) = 344.28, p < .001; χ2/gl = 2.61; GFI = .93; AGFI = .92; CFI = .87; NNFI = .81; SRMR = .084; RMSEA = .05, 95% CI [0.045, 0.059], p = .293). As análises exploratórias posteriores sugerem a possibilidade de melhoria dos índices de validade e de fiabilidade da escala total e da sub-escala Caring, com a retirada de itens específicos. Conclusão: Não obstante as fragilidades encontradas no que concerne à fiabilidade e validade da PPOS-P para uma amostra de estudantes portugueses de Medicina, este estudo representa um contributo científico para a adaptação da escala, que pode ser considerada para efeitos de avaliação de atitudes de centração no paciente nos contextos da educação médica e da investigação.
- First day of radiotherapy for women with breast cancer: predictors of anxietyPublication . Grilo, Ana Monteiro; Gomes, Ana Isabel; Monsanto, Fátima; Albino, Daniel; Augusto, Cláudio; Pragana, CatarinaPurpose: Radiotherapy treatment may generate anxiety, especially on the first day of treatment. This study aimed to identify potential predictors of radiotherapy-related anxiety in women with breast cancer before treatment initiation, in terms of treatment concerns and trait anxiety. Methods: This transversal study included 94 patients from one Radiation Oncology Department, who had been diagnosed with primary breast cancer and who had been prescribed external radiotherapy for the first time. Patients completed a Treatment Concerns Questionnaire and the State-Trait Anxiety Inventory (STAI) before the first treatment session. Results: Women identified radiation involved in the procedure and treatment efficacy as major concerns surrounding radiotherapy. No significant differences were found between patients with higher and lower state anxiety scores, or by age, level of education, cancer treatment protocol used, prior information given about treatment, or report of doubts before treatment initiation. In the final model, the combination of trait anxiety (49.1% of the total variance) with two treatment-related concerns, regarding radiation and the duration of treatment (plus 10.8% of the total variance), significantly predicted treatment-related anxiety experienced on the first day of treatment. Conclusion: Our findings highlight the need to identify women with moderate or higher levels of trait anxiety before radiotherapy initiation and to provide them with a more approach to personalized care, adjusted to their specific concerns and susceptibility to anxious reactions. Effective education sessions involving the discussion of specific treatment concerns, and anxiety coping strategies training should be developed and applied in initial interactions with breast cancer patients.
- Parental cognitive dimensions associated with preschool children’s eating habits: an intervention studyPublication . Barros, Luísa; Andrade, Graça; Gomes, Ana IsabelChildhood excessive weight and obesity are a major public health concern from early childhood. Early childhood is an important period of development for developing healthy eating habits, that may be associated with an adequate present/future BMI. There is extensive evidence that children’s food intake is shaped by early experiences, suggesting ways in which parenting practices may be promoting obesity. But what leads parents to endorse healthier or detrimental educational practices and routines needs further study. 1. Perception of children’s weight: parents of overweight or obese children often fail to correctly perceive their children as overweight; failing to recognize their children’s excessive weight may impeach parents from implementing the best educational practices. 2. Concern: relation between the adequacy of mothers perception of their children’s weight and the level of concern - parental concern is be associated with parental practices. 3. Attribution of control: also, if parents do not consider their children’s eating behavior at least partially controllable by them, they may relinquish some of their responsibility in this area. Self-efficacy: evidence linking parental self-efficacy to parent competence and to parenting practices and behaviors; low parental self-efficacy related to the control of everyday behavior of young children may lead parents to abandon more consistent health practices and endorse permissive and inconsistent strategies. We designed 2 sequential studies that aim to contribute to the understanding of cognitive determinants of children’s eating patterns.
- Promoting patient-centered care in chronic diseasePublication . Grilo, Ana Monteiro; Santos, Margarida Custódio dos; Gomes, Ana Isabel; Rita, Joana SantosNowadays, many people around the world are seeing their lives being shattered and even shortened due to one or more chronic conditions. Chronic illness is a dynamic ongoing process that is determined by a complexity of factors. Patient literacy, motivation, emotional well-being, and resources play an important role in patient adaption and are important challenges for healthcare providers. A systematic patient-centered approach that enables chronic patients to play an active role in their condition management and in the decision making process on a day-to-day basis is required. However, some studies show that health professionals do not always guide their actions by Patient-centered orientation, either by personal issues or by professional and/or institutional barriers. The present chapter aimed to provide a comprehensive approach to patient-centered care in chronic disease and offer a structured guideline as a tool for formal academic education in chronic patient-centered care. This chapter is structured into five sections: (1) Chronic disease: the challenge of the twenty-first century, (2) The patient with a chronic disease, (3) Patient-centered care in chronic diseases, (4) Issues and barriers to achieve patient-centered care, and (5) Guide tool for health professionals’ training and education in patient-centered care.
- Using virtual reality to prepare patients for radiotherapy: a systematic review of interventional studies with educational sessionsPublication . Grilo, Ana; Almeida, Bárbara; Rodrigues, Carolina; Gomes, Ana Isabel; Caetano, MarcoPurpose: To understand the impact of radiotherapy educational sessions with virtual reality on oncologic adult patients' psychological and cognitive outcomes related to the treatment experience. Methods: This review was performed according to the Preferred Reporting Items for Systematic Reviews guidelines. A systematic electronic search in three databases, MEDLINE, Scopus, and Web of Science, was conducted in December 2021 to find interventional studies with adult patients undergoing external radiotherapy who received an educational session with virtual reality before or during the treatment. The studies that provided qualitative or quantitative information about the impact of educational sessions on patients' psychological and cognitive dimensions related to RT experience were retained for analysis. Results: Of the 25 records found, eight articles about seven studies were analysed that involved 376 patients with different oncological pathologies. Most studies evaluated knowledge and treatment-related anxiety, mainly through self-reported questionnaires. The analysis showed a significant improvement in patients' knowledge and comprehension of radiotherapy treatment. Anxiety levels also decreased with virtual reality educational sessions and throughout the treatment in almost all the studies, although with less homogeneous results. Conclusion: Virtual reality methods in standard educational sessions can enhance cancer patients' preparation for radiation therapy by increasing their understanding of treatment and reducing anxiety.
