Introdução: As disfunções sexuais femininas abrangem diversas perturbações que causam alterações no desejo, na excitação, no orgasmo e/ou dor. A fisioterapia parece ter um papel crucial no tratamento destas disfunções, existindo diversas intervenções consoante as necessidades apresentadas. Objetivo: Estudar as intervenções fisioterapêuticas realizadas em mulheres com disfunção sexual e os seus efeitos na sintomatologia e na qualidade de vida e averiguar que estudos futuros serão relevantes para a prática clínica baseada em evidência na fisioterapia na saúde da mulher. Métodos: A revisão sistemática foi realizada de acordo com o protocolo PRISMA. Realizaram-se pesquisas nas bases de dados PubMed, Scielo, ScienceDirect, LILACS, PEDro e Web of Science com as palavras-chave sexual dysfunctions, dyspareunia, vaginismus, female sexual dysfuncion, physical therapy e physiotherapy. Incluíram-se estudos randomizados controlados com amostra constituída por mulheres com disfunção sexual, escritos em inglês, português ou espanhol e excluíram-se estudos sem intervenção fisioterapêutica, sem descrição de resultados ou eficácia do tratamento e com valor na escala PEDro inferior a 4. Resultados: Dos artigos identificados, 14 foram incluídos com informação relevante, sobretudo, relativamente a perturbações de dor génito-pélvica ou na penetração,
verificando-se uma carência na evidência das restantes perturbações. A fisioterapia multimodal, o TENS e as ondas de choque têm bons resultados na melhoria da sintomatologia e da qualidade de vida. Conclusão: Os resultados suportam a eficácia das modalidades de fisioterapia como primeira linha de tratamento de disfunções sexuais femininas, sendo que a fisioterapia multimodal aparenta ter melhores resultados. São necessários mais estudos nesta área, com tamanhos de amostras maiores.
ABSTRACT - Introduction: Female sexual dysfunctions encompass several disorders that cause changes in desire, arousal, orgasm, and/or pain. Physiotherapy seems to have a crucial role in the treatment of these dysfunctions, with several interventions available depending on the needs presented. Aim: Study the physiotherapy interventions performed in women with sexual dysfunction and their effects on symptoms and quality of life and find out which future studies will be relevant to evidence-based clinical practice in physiotherapy in women's health. Methods: A systematic review was performed according to the PRISMA protocol. Searches were carried out in PubMed, Scielo, ScienceDirect, LILACS, PEDro, and Web of Science databases with the keywords sexual dysfunctions, dyspareunia, vaginismus, female sexual dysfunction, physical therapy, and physiotherapy. Randomized controlled studies with a sample consisting of women with sexual dysfunction, written in English, Portuguese, or Spanish, were included and studies without physiotherapeutic intervention, description of results, or treatment efficacy and with a value on the PEDro scale below 4 were excluded. Results: Of the identified articles, 14 were included with relevant information, mainly regarding genito-pelvic pain disorders or penetration, with a lack of evidence for the remaining disorders. Multimodal physical therapy, TENS, and shock waves have good results in improving symptoms and quality of life. Conclusion: The results support the effectiveness of physiotherapy modalities as a first line treatment for female sexual dysfunctions, with multimodal physiotherapy appearing to have better results. More studies are needed in this area, with larger sample sizes.
