ABSTRACT: Introduction: Everything that an individual with chronic diseases does to care for their health is associated with a treatment burden. The Treatment Burden Questionnaire (TBQ) is a tool that allows the assessment of this burden, that is, the impact of healthcare on the patient's life, in different conditions and treatment contexts. This impact can negatively affect the patient's quality of life and is therefore associated with behaviors related to therapeutic adherence. Objectives: We aim to perform the linguistic, content (with cultural adaptation), and psychometric validation of the TBQ questionnaire for the Portuguese language, in a sample of patients over 18 years old with at least one chronic disease. Methods: For the linguistic validation, the TBQ, originally developed in French, was translated and then back-translated. After being reviewed by the research team, the Delphi technique was used to reach a consensus on its content, followed by a pre-test. The validation of the TBQ's psychometric properties was assessed through internal consistency indices, theoretical validity, and Confirmatory Factor Analysis. Results: With a sample of 155 patients, we determined a Cronbach's alpha of 0.87 (1. Medication adherence - 0.754; 2. Monitoring: 0.782; 3. Assessment: 0.742), indicating good internal consistency of the TBQ. The results of the Confirmatory Factor Analysis (CFA) demonstrated reliability, construct adequacy, and that the model has a good fit. Conclusion: The TBQ questionnaire in Portuguese, proved to be a valid and reliable instrument, and its use in healthcare services could help identify and overcome potential barriers to treatment adherence.
