Relação entre função ventricular esquerda e os índices de captação miocárdio/pulmão em estudos Gated-SPECT

dc.description.abstractIntrodução – A tomografia de emissão de fotão simples sincronizada com o sinal eletrocardiográfico (Gated-SPECT) é essencial para a avaliação conjunta da perfusão e da função ventricular esquerda (VE) do miocárdio. Objetivo – Investigar a relação entre a função VE e o índice de captação (IC) miocárdio/pulmão direito (M/PD) e M/P esquerdo (M/PE) nos estudos Gated-SPECT com 99mTc-Tetrofosmina. Metodologia – Amostra de 32 pacientes que realizaram estudos Gated-SPECT por indicação clínica, sendo subdividida em dois grupos: Grupo I (GI) – pacientes com a informação clínica de enfarte agudo do miocárdio (EAM); Grupo II (GII) – pacientes com a informação clínica de isquemia. Por cada paciente adquiriram-se imagens estáticas torácico-abdominais e dois estudos Gated-SPECT do miocárdio (protocolo de um dia esforço/repouso). Nas imagens estáticas definiram-se regiões de interesse (Regions of interest – ROI) para calcular os IC. Nos estudos Gated-SPECT utilizou-se o software Quantitative Gated SPECT/Quantitative Perfusion SPECT para calcular a Fração de Ejeção do Ventrículo Esquerdo (FEVE). Efetuou-se análise estatística descritiva para caracterização da amostra. Aplicou-se o teste de Spearman para avaliar a correlação entre a FEVE e os IC por grupo de pacientes. O Teste de Willcoxon foi usado para comparar FEVE em repouso e em esforço. Resultados – Nos estudos Gated-SPECT em esforço não se verificou correlação estatisticamente significativa entre a FEVE e os IC, para GI e GII; em repouso existe correlação positiva estatisticamente significativa entre a FEVE e os IC, para GI; para GII não se verificou correlação. Na comparação dos valores de FEVE em esforço e repouso nos dois grupos constatou-se a existência de diferenças estatisticamente significativas, sendo a FEVE em Esforço<FEVE em repouso. Conclusão – IC associado ao valor da FEVE pode indicar disfunção VE, contudo, não é possível aplicar de forma viável esta dedução para a amostra total que realizou Gated‑SPECT (GI e GII).por
dc.description.abstractABSTRACT: Introduction – Single Photon Emission Computed Tomography synchronized with the electrocardiographic signal (Gated-SPECT) is central to the joint assessment of perfusion and left ventricular (LV) myocardial function. Objectives – Explore the relationship between LV function and uptake indexes (UI) Miocardium/Right Lung (M/RL) and M/Left Lung (M/LL) in Gated-SPECT studies with 99mTc-Tetrofosmin. Methodology – Sample of 32 patients, who realized Gated-SPECT studies, by clinical indication, was subdivided into two groups: Group I(GI): patients with clinical information of acute myocardial infarction (AMI), Group II(GII): patients with clinical information of ischemia. For each patient were acquired thoraco-abdominal static views and two Gated-SPECT myocardial studies (one-day protocol stress/rest). Were drawn regions of interest (ROI) to calculate UI’s M/RL M/LL in static views. In Gated-SPECT studies, was used the software Quantitative Gated SPECT/Quantitative Perfusion SPECT to calculate the LV Ejection Fraction (LVEF). We performed descriptive statistical analysis to sample characterization. We applied Spearman’s test between LVEF and UI by patient groups. Willcoxon test was used to compare LVEF at stress and rest. Results – In Gated-SPECT studies at stress, correlations between LVEF and UIs were not statistically significant, to GI and GII; at rest, it’s a positive correlation statistically significant between LVEF and UIs, for GI; to GII no correlation was found. In comparison of stress and rest LVEF values, in both groups, was deemed to exist statistically significant differences, with the LVEF in stress<LVEF at rest, to GI and GII. Conclusion – UI associated to LVEF value will be indicator of LV dysfunction, nevertheless it’s not viably applicable this deduction for the whole sample, who realized Gated-SPECT (GI and GII).por
